Specific phobias involve an intense and persistent fear of a particular object, situation, or experience. This fear is often disproportionate to the actual level of danger but can feel overwhelming and difficult to control. At The Lotus Psychology Practice, we offer evidence-based psychological therapy to help individuals understand phobias, reduce avoidance, and regain confidence in situations that currently feel distressing.

Understanding Specific Phobias

Specific phobias involve an intense and persistent fear of a particular object, situation, or experience, such as animals, heights, flying, needles, blood, enclosed spaces, or vomiting. While fear is a normal protective response, a phobia is present when the fear is excessive, immediate, and leads to avoidance or significant distress that interferes with everyday life.

 

Specific phobias typically develop when a stimulus becomes associated with threat through direct experience, observation, or learned interpretation. In some cases, a frightening or distressing event may act as a starting point; in others, fear develops gradually through repeated negative associations or heightened sensitivity to anxiety responses. Importantly, phobias are not caused by a single factor and often emerge in the context of normal learning processes that become over-generalised.

Once established, phobias are maintained by a cycle of fear, avoidance, and short-term relief. When the feared stimulus is encountered or anticipated, the body’s fight, flight, and freeze response is activated, leading to an immediate surge in anxiety. This can include strong urges to escape, avoid, or seek safety. Avoidance then reduces anxiety in the short term, which reinforces the belief that the situation is dangerous and should be avoided in the future. Over time, this prevents new learning from taking place, specifically, that the feared outcome is unlikely or manageable.

As the cycle continues, individuals may become increasingly sensitive to cues associated with the phobia, leading to anticipatory anxiety even before exposure occurs. This can further strengthen avoidance patterns and increase the perceived intensity of the fear response. Physically, phobias may involve rapid heart rate, shortness of breath, sweating, shaking, dizziness, nausea, or a sense of faintness. Emotionally, individuals often experience intense fear, panic, or a strong sense of loss of control when faced with, or anticipating, the feared stimulus. These responses reflect an overactivation of the threat system rather than actual danger, and are consistent with the body’s automatic survival response.

How Can Therapy Help Specific Phobias?

Psychological therapy for specific phobias is highly effective and focuses primarily on breaking the cycle of avoidance.

In therapy, we work on:

  • Understanding how phobias develop and are maintained

  • Gradual, planned exposure to feared situations or stimuli

  • Reducing avoidance and safety behaviours

  • Relearning the body’s response to fear triggers

  • Building confidence through repeated, supported exposure

Specific Phobias in Children & Young People

In children and adolescents, specific phobias may present as:

  • Avoidance of certain environments (e.g. school activities, medical appointments)

  • Distress during exposure to feared stimuli

  • Physical symptoms such as stomach aches or crying

  • Reliance on caregivers for reassurance or avoidance

 

Phobias can sometimes limit participation in everyday activities, particularly when they involve common experiences such as animals, injections, or school-related environments. Treatment often involves gradual exposure in a structured and supportive way, alongside family involvement where appropriate. Within our Child & Adolescent Service, we are able to offer specialist assessment and evidence-based therapy to support children.

It may be helpful to seek psychological support if:

  • Fear of a specific object or situation is significantly distressing

  • You avoid situations due to fear, limiting daily life

  • Anticipatory anxiety is impacting functioning or wellbeing

  • The fear feels disproportionate but difficult to control

  • Avoidance is reinforcing the problem over time

This guides a structured, evidence-based and personalised treatment plan.

Therapy Session Discussion

Prior to your first appointment, you will be asked to complete standardised and validated questionnaire measures, which are used to inform the assessment.  A comprehensive psychological assessment is completed across the first two sessions. This helps us to develop a shared understanding of your current difficulties.​ Following an assessment, you will receive a full report, which includes individualised recommendations and resources, as well as an agreed therapeutic plan and individual goals. 

Based on the assessment, we will agree the most appropriate therapeutic approach, tailored to you. Psychologists are trained across multiple therapeutic modalities, and the approaches are integrated based on an individuals unique needs. All therapies at The Lotus Psychology Practice are informed by scientific research and are and in line with National Institute for Health and Care Excellence guidelines.
